POSITION SUMMARY:
The Care Coordinator positions involves direct, written, and telephonic contacts with clients and their service providers to improve outcomes and patient satisfaction and to ensure access to care and needed supports.
DESCRIPTION OF DUTIES:
· Initiates Care Coordination Plans and ensures all required client documentation is up to date and meets all requirements.
· Serves as the contact point, advocate and informational resource for clients, treatment team, family/caregivers, and community resources.
· Facilitates access to appropriate treatment providers through inquiry, referral, coordination and timely follow-up.
· Increases continuity of care through managing relationships and communicating information and/or facilitating delivery of documentation to all providers especially during transitions in care.
· Utilizes person centered approaches including Motivational Interviewing when interacting with clients.
· Adheres to LifeSkills Code of Ethics and all other agency policies.
· Completes all assigned continuing education requirements
· Performs other duties as assigned by Supervisor.
QUALIFICATIONS:
Associates Degree or Medical Assistant certification and/or experience preferred. Work experience in any of the following areas preferred: medical office setting, providing case management, care coordination or working with clients with mental health or substance use disorders. Maintain a valid KY Driver’s license and automobile insurance coverage.
SKILLS AND KNOWLEDGE REQUIRED
The ability to work with persons in a manner that shows respect for cultural differences is needed. Must possess good oral, and written communication skills. Must be highly organized, have proficiency in communication technologies (email, cell phone) and personal communication skills. Experience with EHR/EMR is desirable
