Legends Health Care is Hiring an Activity Director Near Sandusky, OH

POSITION: Activity Director

SUPERVISOR: Facility Administrator

QUALIFICATIONS:

1. One year of experience in a social or recreational program and in a resident activities program of a health care setting.

2. Certified by the National Certification Council for Activity professionals.

3. In lieu of the certification in item 2, may be an Occupational Therapist, Art Therapist, Music Therapist, Dance Therapist, Recreational Therapist or an Occupational Therapy Assistant.

4. Sufficient verbal and written communication skills to perform the tasks required.

5. Must have valid driver’s license with clean driving record.

ESSENTIAL JOB FUNCTIONS:

1. Develops activities providing the opportunity for residents to experience sensory input-touch, smell, taste, etc., group interaction and personal achievement.

2. Evaluates each resident according to his/her background, interests, abilities, physical, and mental limitations and needs. This shall serve as the base from which the individual activity’s program shall be developed. Provides accurate data as part of the MDS interdisciplinary team.

3. Documents/evaluates the individual’s activity program using the appropriate assessment forms to be placed in the medical record, completing the activity care plan within seven (7) working days of admission.

4. Maintains timely progress notes specific to the resident’s activity plan of care, recording at least quarterly in the medical record and more frequently when appropriate.

5. Develops appropriate records, which indicate resident’s attendance and participation in the program with reference to resident response to the program.

6. Develops a method to implement programs within a designated budget, allocated by the Facility Administrator.

7. Attends and participates in staff meetings, department head meetings, designated committee meetings and resident care plan conferences. Also, shall attend mandatory staff meetings.

8. Coordinates and supervises special events, field trips and encourages residents to remain active members of the community.

9. Coordinates resident council meetings and ensures follow through on appropriate forms reviewed and signed by the Facility Administrator.

10. Provides transportation services to wheelchair bound and ambulatory residents in Company vehicles. This includes medical appointments and off-site activities. Training will be provided on the basic operation of the company vehicles. Must retain working knowledge of the resident van operation, wheelchair lift and all other safety features in the vehicle.

11. Maintains department budget.

ADDITIONAL DUTIES:

1. Encourages residents to make decisions, participate in planning, assumes some degree of responsibility and independence.

2. Establishes an active volunteer program, which includes the screening, training, and supervision of volunteers when appropriate.

3. Interprets to residents, other staff members and the outside community the purpose and achievements of the activity program through in-service training and presentations.

4. Develops a method for obtaining current knowledge of federal and state regulations pertaining to the activities programs.

5. Develops methods for effective utilization of community resources.

6. Serves as a facility liaison to promote positive community support.

7. Attends all company Activity Director meetings, educational meetings, etc.

8. Ensures that nursing service is aware of necessary physician orders.

9. Is responsible for follow-up action reports by consultants or Corporate Quality Assurance personnel.

10. Remains current in activity profession through attendance at continuing education programs. Maintains sufficient CEU’s to keep NCCAP certification current.

11. Is responsible for all company-required reports.

12. Maintains activities calendar.

13. Performs other duties as assigned..

PHYSICAL REQUIREMENTS:

1. Frequently makes rounds and visits patients in their rooms.

2. Frequent pushing and pulling.

3. Occasional bending, squatting, reaching, and grasping.

OCCUPATIONAL EXPOSURES:

Duties may involve exposure to blood and body fluids. This exposure may include infectious diseases. There may also be exposure to air contaminants (including tobacco smoke), and chemicals (including irritants) which require special handling. Attendance is mandatory at initial and periodic in-services provided by the facility regarding the prevention and control of such exposures.
