Hello! Welcome to Legend Healthcare & Rehabilitation of Greenville! We are a beautiful Nursing Facility located in the wonderful city of Greenville. We are looking for talented people to join our fun team. Right now we are needing to hire a Nurse Case Manager. We are looking for someone who is passionate about healthcare and customer service. We need a team player, someone who is energetic and great at communicating with residents and co-workers. Experience is not required, but encouraged. This position is a full time position, Monday-Friday from 8am-5pm.
Here are some of the responsibilities you would have:
- Reviewing incoming referrals
- Assist with admission process and care plans
- Keep insurances, CMs, and physicians updated as needed with plan of care.
- DC planning for short term residents only.
- Ensure Rehab residents are having all their needs met.
- Be willing to assist the charge nurse on the floor if needed.

Our mission is to dignify and transform post-acute care.

Pay: $23.00 - $26.00 per hour
