The Case Manager will maintain client chart records and documentation uploaded into the electronic health system. Track and ensure medical requirements for clients are obtained in the allotted timeframes. Communicate with prescribers as necessary and schedule monthly medication monitoring and work with outside providers. Worker with providers on trainings, medical appointments, and general support. Participate in treatment team meetings, admissions, and discharges. Support clinical staff with rotating on-call schedule. Position is full time and includes the following:
Education/Experience:
Bachelor’s degree in mental health field, social work, or related field. Case management services required.
