We are seeking Case Managers to work in our Assertive Community Treatment (ACT) program. The ACT Case Manager under general supervision performs a wide range of tasks in the community to assist consumers in regaining control over their own lives while promoting the recovery process. Utilizing psychiatric rehabilitation interventions provides skills training, education and case management to consumers and their families. The ACT Case Manager is responsible for monitoring consumer progress towards treatment goals. Extensive documentation required; accurately charts services and consumer progress. Must be able to work well with community agencies and function as part of a team.
