Learning Services, a Collage Rehabilitation Partners is looking for a Part Time Activities Coordinator to join our outstanding team in our Escondido, CA. ReMed, a Collage Rehabilitation Partners is a growing organization dedicated to excellence, honesty and ethics in the treatment of our clients and each other. For more than 35 years, we’ve developed programs to meet the diverse needs of clients with moderate to severe traumatic and acquired brain injuries. We provide treatment for post-concussion syndrome, stroke, memory loss, neurobehavioral issues and accident or sports-related head injuries. Our CARF accredited, post acute residential, vocational and outpatient programs are offered in the Philadelphia, Pittsburgh regions of Pennsylvania, New Jersey, Louisiana and Maryland, Georgia, North Carolina, Virginia, Texas, Utah, Colorado, and California

Responsibilities

The Activities Coordinatorposition is responsible for assisting the Program Specialist in the development and implementation of culturally sensitive recreational, leisure, health-oriented, educational and pre- vocational activities for groups. The position supports client participation in groups and activities to enhance the individual’s stable activity plan. This position provides direct client support, assists in goal and strategy development, data collection and documentation as well as personal care and hygiene support as needed.

Essential Duties & Functions● Displays flexibility and willingness to respond to changes in operations or program needs by changing program assignments when necessary or assuming other duties as assigned. ● Demonstrates skill to support clients in difficult situations (e.g. refusal of activities, medications, and strategies).● Demonstrates ability to identify changes in clients’ behavioral, medical, emotional, cognitive, and physical condition and alert applicable team members and provide care as needed.● Provides minimum to maximum assistance and/ or supervision to promote the client’s success while attending the ReMed Activity Program. ● Develops, organizes and implements group programming that is culturally sensitive and designed to enhance cognitive stimulation, promote socialization and provide entertainment for individuals and groups.● Works in collaboration with the Program Specialist to implement, follow-through, monitor, and provide feedback on effectiveness of ISP/Treatment Plan, systems and/or strategies.● Demonstrates an understanding of the needs of older adults and participates in ongoing trainings concerning these individuals.● Takes responsibility for seeking out current information regarding each client’s program and ReMed information (e.g., RCM weekly updates, meeting minutes, etc.);● Ensures consistent implementation of strategies and assesses use in the program setting; provides feedback to applicable team members as clients’ needs change and/or strategies need revision.● Contributes to the writing of quarterly reports and annual/semi-annual individual care plans according to established guidelines, time frames and licensure requirements.● Assesses, develops, and monitors as needed, adaptive equipment and/or compensatory strategies to increase ability to participate in and/or increase independence in individual tasks or group activities.

● Participates in problem solving with the team and supports the clinical team decision making process.● Maintains thorough, accurate, and timely completion of required documentation (data collection sheets, Credible log notes)● Demonstrates compliance with governing policies and procedures (CARF, ODP, DHS, PDA, etc.).● Maintains standards of cleanliness, maintenance and other health and safety related requirements.

Qualifications

Education & ExperienceA bachelor’s degree and five years of experience working with persons with disabilities is required. Skills necessary to work with older adults and individuals with functional impairmentsis also required, ACBIS certification preferred. Demonstrated ability to develop and run group activities in addition to providing direct client care as needed. Demonstrated planning, organizational, writing and time management skills are required. The ability to demonstrate positive client interaction skills is essential. Physical Requirements & Working Conditions ● Regularly required to communicate, both orally and in writing; occasionally required to speak on the telephone for extended periods of time.● Occasionally required to sit; regularly required to bend, stoop, and kneel; frequently required to stand and walk. ● Regularly required to use hands to type, grasp, handle, or feel; reach, push or pull with hands and arms; and talk or hear. ● Specific vision abilities required include close vision, color vision, and ability to adjust focus.
