Laurel Health Care Company is Hiring a Case Manager Near Dayton, OH

Are you an experienced case manager looking for a new challenge?

As the Case Manager for several skilled nursing facilities, you will manage the prior authorization and utilization review process for new and readmitted skilled nursing facility patients for multiple locations. You will oversee case management activities for The Laurels of Kettering, The Laurels of Huber Heights and The Laurels of West Carrollton

Responsibilities include:

  • Ensuring timely and accurate submission of initial clinical reviews as indicated, 
  • Identifying, and communicating changes in financial coverage or status,
  • Acting as a liaison between the building and payers.
  • Coordinating and providing care that is safe, timely and effective. 
  • Collaboration with Social Workers, Care Transitions team, Physicians, and other Departments to ensure quality discharge planning needs.

QUALIFICATIONS/REQUIRED EDUCATION/TRAINING 

Experience/Education:

  • Clinical and case management experience preferred
  • Computer proficient with experience in managed care prior authorization processes and programs.
  • Covid 19 vaccination or acceptable exemption required
