Laura Baker Services Association is Hiring a QDDP (Qualified Development Disabilities Professional) Near Northfield, MN

The Laura Baker Services Association (LBSA) QDDP ensures that the necessary programs are created, evaluated using objective standards, and modified as needed to provide active treatment in accordance with each person’s ISP and IPP. The QDDP conducts quality assurance assessments and provides feedback to ensure continuous quality improvement of services. They also function as a member of the Oak Street Services Management Team. This is a full-time position with a caseload of six individuals.

Responsibilities Include:

  • Representing the Association in a positive and professional manner, and adhering to the policies and procedures of Laura Baker Services Association
  • Communicating in an open and professional manner within the households, with clients and their families, LBSA staff, support staff, other professionals, and members of support teams
  • Evaluating and providing input on staff evaluations and ongoing feedback to staff members
  • Communicating information, providing support, and serving as resource to clients and staff
  • Evaluating compliance with licensing and regulatory standards
  • Developing recommendations for programming, considering input from clients and staff and revising goals and behavior programs as needed, using a variety of teaching techniques, and incorporating the LBSA philosophy
  • Monitoring the accomplishment of outcomes for each client as specified in the IPP (Individual Program Plan), and taking action necessary to facilitate the accomplishment of outcomes
  • Creating/maintaining CSSP (Coordinated Services and Supports Plan) to ensure compliance with provider responsibilities.
  • Communicating to the Internal Review Committee as needed regarding the restrictions of rights
  • Analyzing incident reports (GERs) in a timely fashion, making note of patterns in behavior, and recommending changes in individual programming, and/or other service delivery methods when needed
  • Analyzing risk and writing Abuse Prevention Plans and service assessments in cooperation with Household Director
  • Reviewing program data (daily logs, goal data, quarterly reports, and annual reports) in a timely manner, and making explicit and knowledgeable recommendations for programming changes, when needed.
  • Evaluating and identifying general and specific staff training needs
  • Instructing staff on a variety of topics, including but not limited to IPP implementation and attending staff meetings as needed
  • Investigating and receiving maltreatment reports as assigned by schedule or by the Program Director
  • And other duties as assigned by the Director of Oak Street Services (DOSS)

Requirements:

  • Must be pass a Minnesota Department of Human Services background study and be able to maintain eligibility.
  • Bachelor's Degree in a related field
  • Minimum 1 year experience working with developmentally disabled individuals.

Minimum starting wage for this position is $19.00 hourly. Starting wage is dependent on experience. Employees are eligible for a raise after the first 6 months. Benefits are available after the first 60 days of employment to all employees consistently working 30 hours per week or more. Benefits include medical, dental, life insurance, 401k plan, and paid time off.
