Community Care Hospice, a collaboration of several non-profit senior living communities, is seeking a compassionate and dedicated Part-time Licensed Master Social Worker (LMSW) to join our team!
Summary:
As a Social Worker, you will play a vital role in providing exemplary end-of-life care to residents in Saline, McPherson, Marion, Reno, and Harvey counties. Partnered with Midland Care Connection, a renown Kansas hospice provider with 45 years of hospice expertise, we are committed to delivering the highest quality of care. In this role, you will assess the social and emotional needs of our patients and their families, develop appropriate care plans, and coordinate the necessary resources and services to ensure their comfort and well-being during their end-of-life journey.
Employment Status: Full Time or Part-Time
Pay: Starting at $26 /hour (pay is based on requirements and like experience)
