• You are passionate about helping people live their best lives
• You understand the social-psychological aspect of care in the total-health approach
• You are positive and energetic—you are a great listener and people like interacting with you
• You are a leader and work well within the team
• You are tactful, driven, and can handle competing priorities
• Experience working with residents in a long-term healthcare setting acceptable in lieu of bachelors degree in social work

What You’ll be Doing

• Working as part of the social services department in scheduling plans of care, documenting progress, and coordinating medical appointments for residents
• Explaining cares and progress with both residents and their families in accordance with both established procedures and HIPAA compliance
• Working with residents and staff to resolve resident concerns through the resident-grievance process
• Assisting with family visits and with activities as needed
• Occasional weekends required, but will be scheduled in advance
