Full-Time Health Care Navigator Nurse (RN) Overview: The Full-Time Health Care Navigator utilizes a collaborative process of assessment, planning, implementation, and evaluation, to engage, educate, and influence members in decisions related to achieving and maintaining optimal health status for various diagnosed chronic conditions.

Essential Duties and Responsibilities of the Full-Time Health Care Navigator Nurse (RN):

  • Using department tools and information/data review, use knowledge and experience of chronic diseases and lifestyle risk behaviors to conduct comprehensive evaluation of member's health status and lifestyle risks.
  • Utilize assessment techniques to determine member's level of health literacy, technology capabilities/access, and readiness to change.
  • Provide through telephonic communication, personalized, individual attention to patients identified with chronic conditions.
  • Encourage member to participate with their provider actively and knowledgably in healthcare decision-making.
  • Utilize motivational interviewing skills to ensure maximum member engagement and promote lifestyle and behavior changes to achieve optimum level of health.
  • Work with patients on self- management support:
    • Help members set long-term and short-term goals for management of chronic conditions.
    • Assess the patient's readiness to change, the importance of change, and confidence in ability to change.
    • Help patients identify and overcome barriers.
  • Provide up-to-date healthcare information regarding specific health care skills and general disease concepts to help facilitate the members understanding of his/her health status.
  • Meet on a regular basis with other Health Care Navigator nurses, attend team meetings as coordinated by management, for information sharing and continuing education activities.
  • Inform patients of incentives tied to participation and able to answer or direct members for help regarding benefit questions relative to the Chronic Disease Management program.
  • Enter timely and accurate documentation/data into the Disease Management database necessary to communicate patient needs and to ensure complete documentation of phone calls.
  • Have flexibility to work some evenings until 9:00pm EST as scheduled.
  • Perform other work-related duties as assigned.

Competencies, Knowledge, Skills and Abilities:

  • Bilingual in English/Spanish a plus.
  • Registered Nurse with a current, unrestricted license.
  • Strong clinical background - preferably 3-5 years of experience in acute care or skilled or rehabilitative clinical settings.
  • Polished communication skills both oral & written. Speaks clearly and persuasively in positive or negative situations. Write clearly and informatively.
  • Good data entry and basic computer skills, proficient in Microsoft Word, Excel & Outlook, with the ability to quickly learn new applications.
  • Able to work independently and as a part of a team.
  • Able to be flexible, well organized, and able to multi-task.
  • Ability to work in a proficient and timely manner to meet production expectations, while maintaining quality in both communications with patients and documentation.
  • Professional business demeanor.
  • Strong work ethic and ability to maintain confidentiality.

A background in Case Management and/or Precertification is not required, however would be a definite plus. This is a work from home position.
