Description
Using our case management model and input from the interdisciplinary team you are prepared to set realistic goals and achieve positive patient outcomes!
Our RN Case Managers’ duties are designed to help you succeed in providing one on one patient care in the security of our patient’s home.
- Responsible for patient assessment, care delivery and coordination, communication and collaboration with patient’s physician and KVNA’s interdisciplinary team – PT OT ST
- Health promotion by teaching and training family members on proper home care procedures and strategies
- Develop home health aide plans of care and provide direct supervision

Requirements
- Valid WI RN License
- Current CPR certification
- Med/Surg or ER experience preferred
- Active Drivers License and reliable insured transportation
