Job Responsibilities:

· Scanning and upload chart documents to electronic health record.

· Responds to internal and external requests for medical records.

· Answers and directs medical record department telephone calls.

· Interacts with hospital and other facilities to coordinate medical record needs.

· Collects patient paper documents within clinic and upload them to electronic system.

· Utilizes tracking system for pathology received.

· Maintains patient confidence by keeping patient records information confidential.

· Maintains professional appearance and conduct at all times.

· Performs other duties as assigned.

Education and experience: High school diploma or general equivalency diploma (GED). Medical facility experience preferred.
