Johnson County Community Health Services is Hiring a Physical Therapist - Home Health Near Warrensburg, MO

Home Health Physical Therapist.

This position will perform home health Physical Therapy visits for the agency.

Work as a part of our interdisciplinary team to help individuals improve and maintain a healthy independent quality of life.
Exhibit Teamwork, flexibility, and professionalism

Responsibilities:

Complete evaluation of the patient to determine treatment plan, goals, and rehabilitation potential to assist the physician in the evaluation of a patient’s level of function and in the development of the plan of care.
Observe and report to the physician as indicated by patient condition, revising care plan as needed for physician’s approval.
Performs physical therapy services/treatments authorized by the patient’s physician that are reasonable and necessary to the treatment of the patients’ illness or injury, or to the restoration of maintenance of function affected by the patient’s illness or injury.
Instruct patient, family members in the rehabilitative program and in the use of required equipment.
Provide current clinical documentation in accordance with established procedures in a timely manner as required by state and federal regulations.
Keep other professional staff informed as needed on patient progress/needs.
Delegates and supervises, on a case-by-case basis, physical therapy services/treatments performed by qualified physical therapy assistants in accordance with Agency policies.
Participates in the agency QAPI Program.
Attends continuing education programs or planned in-services.
Attends patient care conferences and quality of care meetings as required.
Provide in-service training and consultation to Agency health personnel as needed.
Complies with Agency’s orientation and annual health requirements.
Must meet continuing education requirements for certifications.

Minimum Qualifications:

Graduate of a physical therapy program approved by the American Physical Therapy Association or the Council on Medical Education and Hospitals of the American Medical Association, or the Council on Medical Education of the American Physical Therapy Association in collaboration with the American Physical Therapy Association.
Holds current liability insurance, if contracted
Holds current Missouri registration.
Maintains professional skills and knowledge through in-service or continuing education to meet compliance with the Agency’s Education Plan.
On the Family Care Safety Registry maintained by the Missouri Department of Health and Senior Services.
Successful fulfillment of criminal background and Employee Disqualification Lists check requirements.

Physical Requirements:

Must be able to hear and speak in a manner understood by most people.
Must be able to stoop and bend.
Must be able to lift and carry effectively so as to be able to perform the above-listed functions.
Must be able to travel to prospective patients’ residences.
Must be able to carry equipment weighing up to 40 lbs. up and downstairs.
