Jewish Family & Children's Service is Hiring a Care Coordinator Near Glendale, AZ

Overview

Care Coordinator - Children's Behavioral Health: The Care Coordinator will provide support services for children receiving outpatient services who briefly require psychiatric inpatient care. This position will work directly with the child & family, as well as inpatient, outpatient, and regional behavioral health authority (RBHA) staff.

Responsibilities

  • Monitors treatment plans, assists in coordinating discharge of clients, follow-up visits, and assisting clients/families in improving or restoring stability in the community.
  • Coordinates services required to meet the client's whole-health needs using available resources to promote high quality, cost-effective outcomes.
  • Inquires about the client’s health and wellness needs to assist in coordinating primary care or other supportive services with appropriate staff or external parties.
  • Collaborates with regional behavioral health authority in regard to psychiatric hospital admissions.
  • Demonstrates a positive and strengths-based attitude of teamwork.
  • Provides a solution-based orientation in addressing the client & family’s ongoing needs.
  • Collaborates with client/designated representative/guardian, inpatient treatment team, outpatient treatment team, and family as appropriate.
  • Advocates on behalf of the client in order to aid a successful transition to the community from the psychiatric inpatient setting.
  • Monitors the client’s progress.
  • Conducts home visits to observe client's environment and reengage in services, as needed.
  • Conducts follow-up in-person or telephonically; this can occur at the inpatient setting, outpatient setting, place of residence, or in the community.

Qualifications

-Must meet one of the following education/experience requirements:

  • High School Diploma/GED required and 4 years of Behavioral Health experience
  • A.A. Degree and two years of Behavioral Health experience
  • Bachelor’s Degree in Social Work, Counseling, Marriage and Family Therapy, Psychology or related field, with some related behavioral-health experience.

  • Level 1 Fingerprint Clearance Card without Restriction(s)
  • Meet Agency requirements for MVR (Motor Vehicle Record) report
  • Reliable transportation

  • AZ auto insurance
