Palm Vista Nursing and Rehabilitation Center provides a range of services, including skilled nursing care, post-acute medical and rehabilitation care, hospice care, and rehabilitation therapy.

We have an opening for a Social Services Director to develop, implement, and oversee our social services programs.The ideal candidate has social work experience in healthcare, or working with people with disabilities, seniors, or housing.

Responsibilities Include:

  • Conduct biopsychosocial evaluations and establish psychosocial care plans and interventions for residents.
  • Provide discharge planning and benefits counseling for short-term residents and their families.
  • Provide adjustment support and conflict resolution to long-term residents and their families.
  • Schedule and conduct resident/family interdisciplinary care plan meetings as needed.
  • Coordinate with providers for psychiatry, psychotherapy, vision, and dental care.
  • Act as center grievance officer and promptly review and respond to resident/family grievances.

Requirements:

  • Master’s degree in social work or human services field (Bachelor’s degree with experience will be considered).
  • 2-3 years of supervised social work experience working in a healthcare, seniors, or housing focused setting.
  • Passion for improving the quality of life of seniors or individuals with disabilities.
