InterCommunity, Inc is a FQHC LA (Federally Qualified Health Center Look Alike). Our health center provides same-day primary care services to every member of the community, regardless of life situation or ability to pay. We also provide a wide array of behavioral health services. Our community Health Care Centers are located at 281 Main St. East Hartford, 16 Coventry St. Hartford and 828 Sullivan Ave., South Windsor. InterCommunity’s Addiction Services offers a similarly wide range of substance use treatment and recovery-supportive services, with many sites in the Hartford area. We provide services across the spectrum of care, including primary care, residential detoxification and substance use treatment, multiple levels of residential care, outpatient substance use disorder and mental health services for adults and children, intensive outpatient programs, employment and community support, mobile crisis evaluations, judicial support services and social rehabilitation.

Summary:

Responsible individual able to carry out tasks as delegated by the nurse. Responsible for safety of the therapeutic milieu and assisting all clients with ADLs. 

Essential Duties & Responsibilities:

  • Responsible for maintaining safety of the therapeutic milieu through obtaining vital signs and reporting concerns to the nurses in a timely manner. 
  • Performs hourly observations in a timely manner.
  • Completes accurate admission and discharge progress notes before end of shift.
  • Provides thorough search of client and belongings on admission and secures clients belongings appropriately. 
  • Ensures clients receive all possessions and medications upon discharge. 
  • Updates 24-hour book and census report.
  • Assists clients with ADLs as needed.
  • Maintains cleanliness of the unit, kitchenette, clients’ rooms, and triage changing area.
  • Thorough shift to shift report received and given before and after each shift.
  • All other duties as assigned. 

Requirements

Education &/Or Experience:

A current active CNA/MA Certification for the State of Connecticut
