Description

Integrated Life Choices (ILC), a provider of services to people with developmental disabilities, is looking for a part-time Licensed Practical Nurse to join our growing team.

LPN oversees the integrity of all medical related aspects of the assigned ILC caseload of individual supported including medications, healthcare plans, appointments, pharmacy relations, and quality reviews for all individuals ILC supports by performing the following duties. LPN will also assist the Registered Nurse in health care plans, g-tube competency visits and medical tracking.

Duties and Responsibilities include the following: (Other duties may be assigned)

1. Responsible for being on call, as assigned and agreed to, to answer any medical or medication questions/concerns. 
2. Teach QMAP class once a week or as needed. 
3. Tracks and assist case managers in processing medical appointments, annuals, and consultation forms. 
4. Enters and adjusts medications for established individuals that ILC supports. 
5. Monthly MAR approvals for individuals we support. 
6. Acts as liaison between Physician offices, pharmacies, and nurses. 
7. Maintains all confidential records. 
8. Track annual and specialty appointments, and send out appropriate documents to Host Home Provider / Person Care Assistant / Family Care Givers' for upcoming appointments.

Requirements

• Be at least 19 years old
• Possess a current CO LPN License
• Possess a valid CO Driver’s License
• Have current Proof of Vehicle Insurance
• Competently pass and maintain currently trained status in all ILC of CO required training's
• Successfully complete and pass all background checks

 Rate of pay is $35-$40 an hour.
