POSITION SUMMARY

Independent Living Systems is committed to enhancing quality of life through innovative health and support solutions that reduce costs, foster independence, improve compliance, and enhance coordination of care. Central to the effectiveness of ILS-CAre Teams, the Housing Navigation Specialist supports the high-quality care coordination services provided to Health Home Program Members in a compassionate, effective, timely and professional manner.

ESSENTIAL FUNCTIONS

  1. Identify and locate housing options suitable for households as needed.
  2. Develop support systems to meet individual needs; coordinates and facilitates move-in activities, rental negotiations, and service referrals as required.
  3. Foster working relationships with property management firms, property owners, housing services agencies and housing advocacy organizations.
  4. Connect with landlords and familiarize them with the HHP program and community resources to assist both tenants and landlords in the event of potential problems leading to eviction.
  5. Support strategies and systems to meet individual members’ housing needs.
  6. Connect with landlords and familiarize them with the HHP program and community resources to assist in preventing and addressing tenant issues.
  7. Maintain the housing database to support linkages to available housing.
  8. Provide outreach and engagement services to homeless and housing at-risk members, with emphasis on those who are chronically homeless.
  9. In collaboration with CMs contribute to assessments and individualized care plans, as needed, to address member’s housing needs.
  10. Provide information, referrals, crisis intervention, and assistance with permanent housing placement to Care Managers, Care Assistants and other team members as needed.
  11. Provide service linkages and referrals to supportive services such as job developers, social workers, and health care providers, in collaboration with CMs
  12. Document and maintain all case files in eCare, the Health Management Information Systems (HMIS) or other identified databases, in an accurate and timely manner and according to guidelines and workflows.
  13. Maintain compliance to policies and procedures (P&Ps), HIPPA requirements, workflows, timelines and other program requirements.

EDUCATION AND EXPERIENCE

  • Associate degree in health care related field preferred.
  • Prior experience in a health care setting
  • Understanding of Medical Terminology

  • Self-disciplined with excellent analytical and problem-solving skills
  • Technological proficiency
  • Ability to work well with others, including staff, clients, members and the public.
