Illumination Foundation is Hiring a Medical Coordinator (LVN/Vocational Nurse) Near Sylmar, CA

Homelessness is the largest social and public health crisis in California. Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IF currently has 13 facilities with 22 micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.

Illumination Foundation is looking for individuals to join our team and make a real difference in people's lives. This position is eligible for a $1,000 SIGN ON BONUS!

Job Description:

Medical Coordinators are critical to managing the discharge of homeless patients from hospitals and overseeing their discharge care plan. This position will help the Recuperative Care team provide high quality medical follow up coordination and social care.

  • Performing initial intake to ensure proper client discharge from hospital with all appropriate needs for the continuation of care, entering and documenting all findings.
  • Conducting morning, afternoon, and evening checks for all assigned clients to observe well being, ensuring there is no change in medical conditions since hospital discharge.
  • Linking clients to medical services when necessary (insurance, PCP, specialists, referrals)
  • Coordinating home health visits and follow-up MD appointments.
  • Educating clients on illnesses, medications, preventative measures, and overall general health
  • Entering client information into KIPU to maintain client records
  • Exiting clients and preparing client reports for billing
  • Performing administrative tasks such as answering phones and filing paperwork
  • Collaborating with service providers to bring new resources for programs
  • Transporting clients to and from medical appointments if needed to advocate for client needs.

Minimum Qualifications:

  • Current and non-restricted LVN license in California
  • 2 years relevant experience.
  • Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets.
