Case Manager: Care Coordinator

Full-Time

Salary: $17.07 - $23.16 per hour (Dependent upon Experience)

SUMMARY: Care Coordinators maintain responsibility coordinating and providing services for adults/children who have been diagnosed with a mental illness. This position works with a multi-disciplinary team of internal and external providers. This position will focus on assisting with the intake process of clients and triaging client crisis calls.

DUTIES: Accept assigned referrals and engages clients to initiate services. The care coordinator assists in obtaining necessary assessment data, including vital signs and participates in integrated treatment coordinating which may include, treatment plan, releases of information, progress notes, and referrals to internal and external providers. The care coordinator arranges meetings with client and other providers and coordinates needed client services in areas of mental health, chemical dependency, housing, financial benefits, insurance benefits, employment, transportation, education, and other basic needs.

MINIMUM QUALIFICATIONS:

B.A. degree in human services plus 2000 hours of supervised experience (1-year) in adult mental health setting.

DESIRED QUALIFICATIONS: Two years of related experience. Experience working with adults who have been homeless or at risk for homelessness, indigent, have chemical use/abuse issues, and have been involved with legal system.
