Position Summary The Home Health Registered Nurse Case Manager is responsible for modeling the 3 Company values of Compassion, Integrity, and Excellence, and for promoting the Compassus philosophy, using the 6 Pillars of success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Home Health Registered Nurse Case Manager functions as an Interdisciplinary Team (IDT) member to provide routine and emergency assessment (including on-call responsibilities), educational, and evaluative services to meet the needs of patients and their families. S/he helps the patient and family maintain their maximum level of comfort and coping. S/he flexes schedule to meet agency-staffing needs and provides after hours emergency support to the patient and family. S/he is responsible for applying the rules and regulations of state and federal regulatory agencies and other certified agencies. Position Specific Responsibilities • Supervises the care given by Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs). 0% • Monitors productivity of LPNs and HHAs' clinical outcomes. 0% • Manages a larger caseload across the nursing staff. 0% • Supports agency and corporate policies, goals, and objectives. 0% • Promotes a positive working relationship between agency and corporate personnel. 0% • Attends and participates in staff meetings, educational programs and community events, as requested. 0% • Communicates identified needs and potential solutions to supervisor. 0% • Performs routine and emergency assessments. 0% • Completes routine and emergency assessments on each patient as indicated by departmental policy. 0% • Coordinates clinical and all disciplines (Physical Therapist, Occupational Therapist, Social Worker, Speech Therapist, Licensed Practical/Vocational Nurse, and Registered Nurse/Home Health Aide) as indicated by Plan of Care (POC) through case management. 0% • Records observations, treatments, and other pertinent information. 0% • Communicates with IDT, Medical Director and Attending Physician as directed regarding measures to alleviate symptoms, and monitors response to measures implemented. 0% • Provides physical and emotional intervention to support the patient and family in period of crisis through case management. 0% • Prevents unwarranted hospitalizations by meeting patient and family needs in the home. 0% • Develops a patient specific plan of care and coordinates patient care in conjunction with the ordering Physician or NPP. 0% • Collaborates with IDT to meet needs of patient for clinical and psycho-social interventions. 0% • Collaborates with IDT to coordinate home health care for the patient and family to ensure appropriateness, continuity, and quality of care. 0% • Updates the POC with changes in condition, medication and treatment in conjunction with Physician or NPP. 0% • Assists in identifying the need for intervention of other IDT members. 0% • Effectively communicates patient and family needs to IDT. 0% • Educates patient and caregiver regarding care of patient, disease process, symptom control, wound care, and IV therapy. 0% • Admits patients. 0% • Participates in on-call rotation. 0% • Explains Home bound criteria and skilled need to patient and families. 0% • Completes all oasis assessments as required. 0% • Maintains appropriate communication. 0% • Keeps Physician and Medical Director informed of patient needs and condition. 0% • Ensures documentation at bedside. 0% • Notifies other IDT members of physician concerns and updated orders. 0% • Completes and submits clinical notes and Oasis, time logs, and all required documentation by 10 A.M. the following morning. 0% • Regular attendance is an essential function. 0% • Performs other duties as assigned. 0% Education and/or Experience Graduate of an accredited school of nursing preferred. Minimum one year nursing practice or equivalent experience preferred. Home Health/Hospice, Acute or critical care/hospital care experience preferred. Skills Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage. Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, colleagues, investors, and external parties. Strong written and verbal communications. Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces hospice philosophy. Certifications, Licenses and Registrations Registered Nursing License in state of employment. A valid driver’s license and auto liability insurance. State Specific Louisiana Two years' full time experience as a Registered Nurse. Two years’ full-time clinical experience as a Licensed Practical Nurse in hospice shall be sufficient in place of clinical experience as a Registered Nurse. Mississippi One year full time experience as a Registered Nurse or 3 years full time continuous employment as a Licensed Practical Nurse immediately preceding Registered Nurse licensure.
