Palliative Care Nurse Practitioner (NP)
Hospice and Palliative Care, Inc is seeking a Full-time Nurse practitioner to join our Palliative Care program to support patients and their families dealing with chronic/serious illness. We will select a qualified Nurse Practitioner to join our team who exhibits exceptional clinical skills, demonstrates compassion and can support our mission of providing quality care to our patients.
The role requires the Nurse Practitioner to conduct visits in patients’ home as well as office setting. This includes the use of telehealth. The NP will document in the medical chart by recording subjective, objective data, assessment, and plan of care in a consistent, legible, concise, and timely manner. Will provide patient/family education regarding disease processes and utilize prescriptive authority.
The role additionally requires the NP to facilitate/conduct discussions about goals of care and advanced care planning with patients/families. The NP will work in conjunction with primary care physicians to manage symptoms including pain, provide guidance, support, and develop a plan of care.
Participates in community/professional outreach activities as requested. Attends and participates in committee, staff, and patient-related meetings as requested. Coordinates care with patient’s primary care provider, the A.I.M. team members, community agencies, and patient/family. Minimal on-call responsibility.
Must be a current NYS licensed Nurse Practitioner , must have NPI number and be Medicare Enrolled with Medicare Identification number or be eligible for Medicare Enrollment within 6 months of hire. Eligibility criteria for position requires National Certification through AANP or ANCC, and a Master’s degree. Minimum two years of experience as an advanced care provider preferably in homecare, palliative or have experience working with patients with chronic illness. Must be flexible, willing to travel (paid mileage).
