RN Case Manager - Hospice of North Ottawa Community
JOB STATUS: Full time, 80 hours every two weeks
Job Description:
In this role, you will have the ability to:
- Manage the overall care and well-being of patients and their families at end-of-life
- Collaborate with an interdisciplinary team to provide holistic care to our patients
- Access our Health system as a great support network with immediate access to patient care resources
- Serve a smaller, intimate patient base
- Operate within a radius of services within 30 miles of Grand Haven

Minimum Requirements:
- Valid MI RN license
- A minimum of 2-3 years of experience as an RN, with some Hospice or Home Care nursing, preferred
- Excellent assessment and critical thinking ability
- Current CPR certification
- Must be able to practice independently
- Working knowledge of adult disease processes
- Must have a valid driver's license, current auto insurance, and a reliable automobile
- Comfortable with computer charting
- Must be self-directed with the ability to work with minimal supervision
- Must be available to work weekday nights and weekends.
As an RN Case Manager, you will provide skilled nursing care for hospice patients in person and via telephone. In this critical role, you respond to patient/family calls as required, assess the needs of the patient/family unit, and plan, implement and evaluate nursing care in accordance with provider orders and the interdisciplinary plan of care. Your care includes administering interventions as prescribed by the provider and educating the patient/family as appropriate. You also ensure that equipment and other necessary items are services are available.
