Honor Community Health is Hiring a Community Health Worker Near Oakland, MI

Honor Community Health is a 501c3 Federally Qualified Health Center co-located within 20 locations. Our mission is to provide for the health and wellness needs of the underserved of Oakland County through the provision of comprehensive, integrated primary, behavioral health, and dental care. Position Description: 

The Community Health Worker (CHW) engage in collaboration with multiple team members of an interdisciplinary team to deliver integrated, person-centered, and comprehensive care to eligible beneficiaries to successfully address the complexity of comorbid physical and behavioral health conditions. This position will work directly with patients and their representatives to assist in the delivery of six core services: comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support, and referral to community and social support services. This position requires tracking and follow up, ensuring details of care are documented in the Electronic Health Record and population health management tool. 

This position reports to the Integration and Care Management Manager. Our standard hours are Monday- Friday, 8:30am-5:00pm.

Responsibilities:

  • Promote self-advocacy by assisting recovering persons to have their voices fully heard; their needs, goals and objectives established as the focal point of rehabilitation and treatment.
  • Assist with day-to-day operations of the health center including rooming patients, scheduling appointments, conducting outreach to patients referred to MAT, arranging transportation,
  • This position requires tracking and follow up, ensuring details of care are documented in the Electronic Health Record and population health management tool
  • Identify barriers (internal and external) to full participation in treatment and recommend strategies for improving access and overcoming barriers.
  • This position will work directly with patients and their representatives to assist in the delivery of six core services: comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support, and referral to community and social support services.
  • Performs other duties as assigned.

  • A minimum of two years’ experience working in a clinical environment required
  • Current MiCHWA Community Health Worker certification preferred.
  • Bilingual (Spanish/English) preferred.
