As a home health Licensed Medical Social Worker, you will be responsible for conducting focused care coordinating agency and community resources for the patient in the home setting. Home health is extremely rewarding and allows to you spend time getting to know your patients and their families.

Qualifications:

• Masters or doctoral degree from an accredited school of Social Work
• One (1) year social work experience in a health care setting
• Current state license
