Responsibilities for Case Manager

• Coordinate and facilitate patient care through assessment, evaluation, planning, and implementation
• Communicate patient needs to a variety of care team members
• Work collaboratively with patients, families, and staff to ensure high quality care

Qualifications for Case Manager

• 2 years of medical background
• Strong written and verbal communication skills
• Must display a high level of confidence
• Must have background in sales

Job Types: Full-time, Part-time

Pay: $21.00 - $25.00 per hour

• 10 hour shift
• 4 hour shift
• No weekends

Supplemental pay types:
