Summary:

Provides primary care diagnosis and treatment in a family practice clinical setting to a specified population. Counsels and educates patients regarding treatment and refers to ancillary and support services.

Requirements

Minimum qualifications:

Certified as a Family Nurse Practitioner (FNP) or Advanced Registered Nurse Practitioner (ARNP) in the State of Idaho or qualified to receive certification as an FNP or an ARNP in the State of Idaho. Minimum 2-4 years’ experience directly related to the duties and responsibilities specified. State of Idaho RN Licensure or ability to receive licensure required. CPR certification or must obtain within 90 days of hire.

Knowledge, Skills, and Abilities Required:

  1. Knowledge of patient care charts and patient histories
  2. Knowledge of health education theory and practice.
  3. Ability to supervise and train employees, to include organizing, prioritizing, and scheduling work assignments.
  4. Knowledge of clinical operations and procedures.
  5. Ability to maintain quality, safety, and/or infection control standards.
  6. Knowledge of current principles, methods and procedures for the delivery of medical evaluation, diagnosis and treatment in area of expertise.
  7. Strong interpersonal and communication skills and ability to work effectively with a wide range of constituencies in a diverse community.
  8. Knowledge of related accreditation and certification requirements.

Duties and Responsibilities:

  1. Provide primary care through diagnosis, clinical judgement, and management to restore, maintain, and improve health status in accordance with practice policies and scope of practice, including:
  2. Assessment of physical, emotional, and developmental status of the individual and family;
  3. Analysis of health behavior related to personality, lifestyle, and culture;
  4. Prescriptive treatment of health problems as defined by nurse practitioner act;
  5. Counseling and client advocacy;
  6. Provides health education to clients, clinic staff, and community organizations;
  7. Performs assessment of the environmental status of the community and its impact on health.
  8. Obtains patient histories and develops patient care charts, ensuring completeness and accuracy.
  9. Supervises and/or coordinates the activities of patient care and/or support staff.
  10. Works in collaboration and/or coordination with other health-care providers within the clinic team and community agencies to provide comprehensive client care.
  11. In coordination with other clinic staff, maintains system of quality assessment and assurance in accordance with quality assurance plan.
  12. Participates in program planning, development, implementation and evaluation relating to specific job, clinic, and program functions.
  13. Participates in a continuing education program designed by the nurse practitioner and approved by the Medical Director.
  14. Participates in staff meetings, conferences, and in-service training and continuing education.
  15. Participates in recruitment, interviewing and performance evaluations of clinical personnel.
  16. Maintains standards of care as described in Principles of Practice, protocols, and Scope of Practice as approved by the State of Idaho Board of Nursing and the American Medical Association.
  17. May participate in planning programs and in developing specific health policies and procedures.
  18. Practices safety, environmental, and/or infection control methods.
  19. Participates in QA/QI activities.
  20. Performs other related functions as required for clinic operations.
  21. Heritage Health staff have an active role in our Patient Centered Medical Home model of care. This role is designated as part of the Heritage Health PCMH Care Team.
  22. Regular and predictable attendance is an essential function of this position.
  23. Performs other job-related duties as assigned.
