helping hands home care service is Hiring a Physical Therapist - Part-Time Near Hermitage, PA

Helping Hands is expanding and looking for Physical Therapist to be part of our company growth and diversification. Helping Hands is looking for part-time or PRN Physical Therapist to serve Mercer and Lawrence counties. 

Overview

The Physical Therapist provides rehabilitative care (physical therapy) for patients needing assistance and training in ambulation, transferring, gait training and home exercise programs. The Physical Therapist works as part of the interdisciplinary home care team including RN, OT, COTA, PTA, SLP, MSW and Home Health Aides.

PRIMARY RESPONSIBILITIES 

  1. Provides physical therapy services to patients according to a plan of care. This may include, but is not limited to:
    1. Assessing and evaluating therapeutic/rehabilitative/functional status and participating in developing the total plan of care.
    2. Assessing for muscle strength, mobility, gait, ROM and transfer capabilities.
    3. Directing physical therapy treatment.
    4. Instructing patients, family and/or caregivers in the use and care of therapeutic appliances.
    5. Determining priority needs for physical therapy.
    6. Reporting patient’s reaction to treatment or changes in care to patient’s physician as needed and appropriate.
    7. Conducting appropriate evaluative procedures; assessing patient’s home environment as appropriate; and identifying patient’s equipment needs.
  2. Initiates physical therapy program and coordinate with other team members, patient, family and/or caregivers in therapy program, as well as the goals of therapy.
  3. Instructs patient, family and/or caregivers and other team members in the patient’s treatment regimen as indicated.
  4. Participates in patient case conferences.
  5. Supervises the Physical Therapist Assistant (PTA), which includes, but is not limited to, the following activities:
  1. Provides direction and instruction regarding the care provided by the PTA.
  2. Performs an on-site supervisory visit at the patient’s home, a minimum, every thirty (30) days or more frequently to guarantee that the plan of care is implemented, followed and that patient/family are satisfied with the care and services being provided. Documents supervisory visit in the patient record.
  3. Conducts regularly scheduled case conferences, either face-to-face or by telephone, and patient record reviews with PTA.
  4. Conducts employee performance evaluations for the PTA at the completion of the probationary period and annually thereafter.
  1. Participates in the agency’s Quality Assessment and Performance Improvement program as requested.
  2. Maintains appropriate clinical records, progress notes and reports to the physician, and submits these records in a timely manner in accordance with agency policy, state and federal guidelines and regulations.
  3. Confers with the attending physician, home care team members and patient, family and/or caregivers to promote coordinated and comprehensive care.
  4. Participates as a member of the Professional Advisory Committee and reports all appropriate and relevant information relative to the provision of clinical services.
  5. Completes a minimum of 12 hours of inservices per year.
  6. Participates in meetings and other related activities as needed.
  7. Meets or exceeds delivery of agency service standards in a consistent fashion.
  8. Adheres to the agency’s Code of Conduct.
  9. Complies with local, state and federal laws, Medicare regulations and established agency policies and procedures.
  10. Interacts with all staff in a positive manner, supporting the agency’s mission.

PHYSICAL REQUIREMENTS

  1. This position requires the physical therapist to work in a variety of environments (housing styles, cleaning and maintenance, heating and cooling) during visits to patients in their place of residence.
  2. Visual/hearing ability must be sufficient to comprehend written/verbal communication.
  3. The therapist must have ability sufficient to ambulate throughout a patient's home, navigating stairs and other physical barriers typical to private residences.
  4. The therapist must be able to operate a motor vehicle and travel to and visit in patient’s homes in varying weather and road conditions.
  5. Must be able to deal effectively with stress.

EDUCATION AND EXPERIENCE

Education: Must possess a bachelor’s or master’s degree in physical therapy approved by an accredited organization: The American Physical Therapy Association, The Council on Medical Education and Hospitals of the American Medical Association, The Council on Medical Education of the American Medical Association and The American Physical Therapy Association.

Experience:

  1. Minimum of two years’ experience as a physical therapist. Community/home health experience preferred.

Licensure, Registration, and/or Certification:

  1. Current and valid license as a Physical Therapist in the Commonwealth of Pennsylvania under the Physical Therapy Practice Act (63 P.S. §§1301-1312).
  2. Valid driver’s license and reliable transportation that is insured in accordance with Pennsylvania requirements.
  3. Satisfactory references from employers and/or professional peers.
  4. Satisfactory criminal background check.
