The Program Coordinator supports the Program Director in overseeing the daily operations of the Home and Community Servicesprogram, in particular ensuring coordination of services for the assigned individuals who receive Home and Community Services from Passavant Memorial Homes, in accordance with each individual's Individual Plan.
