Responsibilities

• Provide skilled physical therapy within the clinic, home and/or school setting
• Complete physical therapy assessments using formal and informal assessment tools
• Develop and implement developmentally/functionally appropriate client-centered goals
• Consistently assess progress and adjust treatment plans as needed
• Maintain timely treatment records through our EMR system
• All signed no later than end of business hours on Friday (6pm)
• Prepare patient reports (evaluation, progress and discharge) within the given timeframes:
• Daily notes – complete same day
• Initial evaluations – complete within 48 business hours
• Re-evaluations – complete on a yearly basis (may be more often based on payer/insurance)
• Discharge – complete on day of discharge
• Provide ongoing parent support and education including home program for carryover skill development
• Collaborate/consult with allied health and education professionals
• Abide by the rules, regulations, policies and procedures of the company

Qualifications

• Master’s Degree in Physical Therapy.
• Must hold a current license in CO
• Assumes responsibility for ongoing continuing education and professional development
