Responsibilities:

Essential Job Duties:

1. Completes nutritional assessments, MDSs and care plans using electronic systems
2. Provides routine and change of condition progress notes in the resident medical records
3. Participated with health care team concerning residents plan of care during care conferences and/or At Risk meetings
4. Checks residents progress toward meeting nutritional goals and determines effectiveness of nutritional care plans; documents outcome in progress notes and adjusts care plans as needed
5. Visits residents regarding menus, food service, food preferences and nutritional care
6. Reviews weight records routinely and communicates variances to the interdisciplinary team. Maintains a spreadsheet of weights
7. Ensure compliance with nutritional standards, government regulations and company policies and procedures
8. Attends and participates in scheduled in service training, educational classes and meetings as required by regulatory agencies and company polices
9. Provides oversight in the maintenance of standards for safe food handling following company policies & procedures, local and state food codes and state federal regulations
10. Performs regular audits of dietary services to ensure safe food handling
11. Attends and participates in routine staff meetings and sits on required committees
12. Assists in writing menus for resident/regional preferences and approves the use of nutritionally equivalent substitutes
13. Assists in developing and updating department policies and procedures
14. Maintains registration as required by the Commission of Dietetic Registration and meets requirements for state licensure/certification

Qualifications:

  • Baccalaureate degree or other advanced degree in dietetics plus internship

  • Registration with the Academy of Nutrition & Dietetics with licensure or certification in the state where he/she practices

  • Two years experienced in long term care preferred

  • Ability to communicate with, understand, empathize with, work with and be tolerant of residents, families, visitors, Food & Beverage staff, nursing staff, physicians, physicians extenders and department heads.
