Home Health Aide / Personal Care Assistant JOB SUMMARY:
A HHA/PCA is a paraprofessional who is specifically trained and competent to perform assigned functions of personal care to a patient in their residence under the direction, instruction, and supervision of a licensed and Registered Nurse (RN).
QUALIFICATIONS:
Compassionate employees who are willing to work with diverse consumers in their home.
Have a sympathetic attitude toward the care of the sick and elderly.
Ability to carry out directions completely through written and spoken communication.
Mature ability to deal effectively with all of the required demands of the outlined job position.
RESPONSIBILITIES
Understands and adheres to established HHHS policies and procedures.
Performs personal care and bath tasks as ordered.
Completes appropriate visit records promptly according to rules, policies and procedures.
Reports changes of pertinent patient's condition and related needs to the RN.
Performs household services essential to health care of a home as assigned.
Ambulates and exercises the patient as assigned.
Performs procedures as an extension of the therapy services, e.g., range of motion (ROM) exercises as assigned.
Assists with administering medications.
Attends in-service and continuing education programs as appointed.
Attends patient care conferences as scheduled.
WORKING ENVIRONMENT
Works indoors and outdoors with patients as assigned
Work with HHHS office staff, field clinicians, and patient in their homes.
May need to travel to and from patient homes.
