Hazelden Betty Ford Foundation is Hiring a Care Coordinator Near Rancho Mirage, CA

Responsibilities

As a Care Coordinator, you will:

  • Assist clinical case managers by working with clients, gathering, compiling and coordinating continuing care information
  • Provide AA/NA contacts in clients home area, set up interviews, arrange transportation and contact agencies with regard to clients continuing care plan
  • Maintain the continuing care database, with special emphasis on continuous updates of clinical value and work
  • Work with the admissions office and provide back up for admitting clients, when needed, as well as provide the continuing care chart checks for all patients leaving treatment
  • Follow up phone calls within 5 days of discharge for WI client facility placements for continuing care services.

Qualifications

Required

  • High School Diploma or equivalent
  • 1 or more years in customer service; intermediate level experience with Word software, including Outlook, Word, PowerPoint; in online research

Preferred 

  • Vocational or associates degree
  • Experience with patient database
  • Basic knowledge of chemical dependency, including 12 step programs and the MN model
  • Experience in a behavioral care setting.
