Position Description

About Harris County Public Health:

Program Specialist Position Overview:

The Program Specialist is responsible for supporting the Texas Asthma Control grant under the Chronic Disease Prevention Program by providing assistance on grant-related goals, objectives, and activities, including a variety of stakeholder and community engagement initiatives. The specialist will be responsible for fostering partnerships, stakeholder collaboration, and strategic planning to support asthma control within the Harris County jurisdiction.

Duties and Responsibilities:

  • Develop initiatives to fulfill programmatic goals and objectives, including meeting grant deadlines and requirements, strategic stakeholder participation, and sustainability planning.

  • Establish and maintain collaborative relationships with community stakeholders, schools, providers, and community healthcare agencies to facilitate and support quality asthma care.

  • Facilitate asthma self-management education (AS-ME) and home assessment sessions for community members from priority settings identified by environmental scan.

  • Recruit community leaders to serve as asthma program facilitators who will administer AS-ME programs in their respective communities.

  • Facilitate trainings for community health workers, healthcare providers, school nurses, and other healthcare professionals on AS-ME and quality asthma care.

  • Attend community outreach events, such as health fairs, forums, and conferences, to support programmatic goals.

  • Assist with gathering and analyzing data to be submitted for monthly and quarterly progress reports.

  • Assist with managing programmatic inventory and supplies needed for each AS-ME and home assessment cohort.

  • Perform other duties as assigned.

Requirements

Education:

  • High School diploma or GED equivalency

Licensure:

  • Valid Driver's License (Texas upon hire)

Knowledge, Skills, and Abilities:

  • Proficient in MS Office Suite (Word, Excel, PowerPoint, Outlook)

  • Excellent communication skills (verbal & written)

  • Ability to establish and maintain effective working relationships and foster teamwork in a diverse environment

  • Ability to maintain confidentiality when dealing with sensitive information

  • Ability to work well with public and staff members

Experience:

  • Six (6) months experience in community engagement, program development, or direct work under public health grants

Language:

  • Bilingual in English and Spanish

General Information

Position Type, and Typical Hours of Work:

  • Full-Time | Grant-funded

  • Monday - Friday from 8:00 am to 5:00 pm

  • Must be willing to flex work hours when required, including weekends.

  • Able to travel within Harris County as needed.

Work Environment:

  • This job operates in both a professional office and a community environment.

  • This job frequently requires visits to Harris County Public Health's various clinic sites including field operations.

Physical Demands:

  • This is a largely active role; some duties may require the ability to life up to 20 pounds and bending & standing for periods at a time.
