You will be working in home-like cottages with 12 elders per cottage and a ratio of 1:4 for CNA to elders on dayshift and 1:12 on night shift. Our nurse to elder ratio is 1:24. The CNAs take care of the elders by cooking food, cleaning rooms and kitchen, assisting with activities, and providing cares. OVERVIEW
Under general supervision by the Director of Nursing, nurses continue to be responsible for all nursing skills including assessment, care planning, evaluations and providing nursing care. They are members of the Clinical Support Team (CST) and work as a teacher, mentor and partner with Shahbazim (CNA’s), therapists, the social worker, dietician, physician, activity professional, and food service manager. They are responsible for all nursing roles and responsibilities within his/her scope of practice while practicing as a Gerontological Nurse. Nurses support two Green House homes (24 Elders) depending on the Elder’s needs and the time of the day. The Clinical Support Team Nurse interacts with all staff in a manner that is respectful and collaborative.
Pay: 45.00/hr (shift differential is an additional 2.00/hr weekdays and 4.00/hr weekends)
Shift 1 : Rotating 12/hour days 6am-6pm
Shift 2: Rotating 12/hr nights 6pm-6am
Shift 3: Part-time Weekend day shift
QUALIFICATIONS
· Current Wyoming RN License without restriction
· Current BLS Certification
· Prefer Gerontological Nurse Specialization and/or experience in working with Elders
SKILLS
· Must be able to stand, squat, kneel, push and pull
· Must be able to lift 30 lbs
· Must be able to interact with customers and co-workers in a friendly and professional manner
· Good attendance, including arriving at work on-time is a must
· May be required to perform other tasks as needed and requested by employer
TRAINING
· Initial Green House CORE/ENCORE Training
· Ongoing in-service training
RESPONSIBILITIES
· Follow standards of practice for nursing care and assures Elder safety at-all-times
· Attends Shahbazim Team meetings upon invitation to provide support and guidance
· Does initial and on-going assessments for Elders admitted, as well as transfers
· Obtains updates and report on each Elder from Shahbazim on each shift
· Visits the Elders during shift to monitor clinical status to ensure clinical care needs are met
· Complete Elder specific clinical assessments; considering any primary diagnosis and associated risks
· Assesses unresolved Elder pain; plans and implements pain management interventions
· Identifies clinical teaching needs of staff, Elders or family; teaches and documents
· Monitors standard precautions-i.e. presence of gloves, soap, towels, etc.
· Administers medications and treatments following the 5-Rights
· Participates in the development and adequacy of all interdisciplinary care plans
· Evaluates and ensures compliance with the care plan
· Monitors dietary needs, prescribed diets, food and fluid intake of Elders
· Maintains and prepares emergency supplies and equipment
· Monitors for pressure ulcer prevention, detection and treatment; according to nurse protocol and orders
· Identifies safety risks/issues and institutes appropriate interventions
· Institutes nursing measures to prevent URI’s per nursing protocols
· Initiates nursing measures to prevent contractures
· Provides staff with integrated care plans for physical and emotional needs of our Elders
· Attends CST meetings to discuss integrated care plans by Elder conditions and behaviors
· Other duties as needed
