Responsible for offering pastoral ministry to the nursing home residents and their family members.Responsibilities include visitation, Bible studies, and worship services.Also responsible for acting as liaison between residents and clergy, and between veterans residing here and veteran organizations.

ESSENTIAL REQUIREMENTS

• Preferably a Divinity Degree from an accredited seminary and at least two units of Clinical Pastoral Education.
• Must be an ordained minister.
• Must have at least two years of experience in a hospital or geriatric setting preferred.
• Must be duly licensed in accordance with laws of this state.

·Employment is contingent on successful completion of a pre-placement physical.

·Employment is contingent on completion of all pre-employment criminal background check and Medicaid Exclusion Screening, both indicating negative results.

ESSENTIAL DUTIES AND RESPONSIBILITIES

·Responsible for providing pastoral ministry to the residents by visiting on an ongoing basis, and as opportunities arise, their family members.

·Available 24 hours a day, upon request by the staff, resident or family member to residents who are ill or dying and their family members.

·Responsible for distributing devotional materials and large print Bibles to residents who request them.

·Responsible for either conducting weekly worship services or inviting local clergy to do so and to administer or arrange administration of Holy Communion at least four times annually.

·Responsible for overseeing and attending Tuesday evening worship services offered by area congregations.

·Conduct Bible studies on each floor twice a month and offer a more intensive study twice a month.

·Conduct hymn-sing once a week with the primary focus on the singing of familiar and other denominational hymns, offers Bible study at minimum twice a month.

·Oversees all worship services and Bible studies conducted here by outside groups.

·Upon request will conduct a funeral service and other services as appropriate

·Conducts quarterly Memorial Service for residents who have died.Staff is encouraged to participate; residents and family members are invited to attend.

·Participates as a liaison to various veteran organizations for those residents who are veterans and have a signed a release.

·Notifies clergy when members are admitted here or have been admitted to the hospital, or request a visit, per request of the resident or POA.

·Weekly updates the list of Catholic residents for the Eucharistic Ministers who distribute communion on the weekend, and faxes a list to the Catholic Church each Thursday. Prior to Mass, the last Saturday of each month, each floor receives an updated list of Catholic residents on their floor.

·Organizes the Veteran’s Day service and other annual holiday services such as and Christmas Eve Services, National Day of Prayer, Ash Wednesday, Good Friday, Jewish Holidays and other religion celebrations as appropriate.

·Attends on occasion the Interdisciplinary Care Conference and functions as part of the team.

·Is subject to falls, burns from equipment, odors, etc., throughout the workday.Is subject to exposure to infectious waste, diseases, conditions, etc., including the AIDs and Hepatitis B viruses.

·Required to be on-call for emergencies.

·Attends all mandatory in-services.

·Responds to requests for chaplain services from staff.

·Ability to establish and maintain effective rapport with residents, families, co-workers, and community agents.Ability to communicate effectively both orally and in writing with individuals on all levels.

·Must be an active participant in the facility’s Quality Assurance/Performance Improvement (QAPI) program and Performance Improvement Project (PIP) and carry out roles and responsibilities as assigned.Must attend trainings for QAPI, follow policies and procedures in support of QAPI efforts, and support QAPI initiatives and findings.Must also report potential or actual non-compliance issues to the facility’s QAPI committee.

·Perform other reasonable duties as assigned by supervisor.
