Grace Healthkare is Hiring a LVN Near Pollok, TX

We're looking for an attentive Licensed Vocational Nurse to join our team at a facility that provides care for elders. The ideal candidate for this position will have prior experience working with physically and mentally disabled nurses and has a basic understanding of care giving. A successful candidate will be responsible for providing patients with daily health screenings, administering medication and subjecting patients to life style changes such as physical therapy and healthy eating habits.

Responsibilities:

  • Provide long term care to nursing home patients.
  • Provide nursing care to patients.
  • Deliver patient care under the supervision of a registered nurse.
  • Provide supportive and rehabilitation services to patients.
  • Work with family members, case managers, and other health care staff to ensure continuity of care for the patient.
  • Follow physician's orders for treatment and medications.
  • Patient intake and outtake.
  • Obtain medical history and perform physical assessments in accordance with established protocols.

Weekly schedule:

  • Monday to Friday
  • Rotating weekends
