Acenda Integrated Health offers a wide range of mental health, crisis services, parenting support, family-focused therapy, and residential programming.

What you'll do

• Attend an all-expenses paid weeklong training in Denver, Colorado.
• Travel to clients' homes and provide home visits to first-time mothers providing education and support through pregnancy and up until the child's 2nd birthday.
• Work to improve pregnancy outcomes, child health and development outcomes, and the economic self-sufficiency of families.
• Build long-term therapeutic relationships with clients and their families.
• Provide community resources as needed per nursing assessment.
• Adheres to the nursing process and the Nurse-Family Partnership model of home visitation.
• Attends and participates in case conferences and team meetings.
• Collaborates with client and family to develop short- and long-term goals.
• Formulates nursing diagnosis based on nursing assessment, client goals and Nurse-Family Partnership outcomes.

What you'll bring to the team

• Bachelors degree in Nursing (BSN) required
• Active NJ Registered Nurse license (in good standing)
• Must have and maintain a valid driver's license, use of an insured vehicle and an acceptable driving record
• Professionalism and a commitment to excellence in care
