GOOD SHEPHERD HOSPICE is Hiring an Intake Specialist Near Farmingdale, NY
The Home Care Intake Specialist will: work closely with Home Care Transition RNs as partners in processing referrals; be responsible for transcribing referral information into the Electronic Medical Records; identify key points on the referral that would be red flags for escalation to RN. The Home Care Intake Specialist will maintain professional relationships with physician groups and hospital referral sources; act as a point of contact at Intake and provide customer service on behalf of Catholic Health Home Care.
- Bachelor's Degree required
- Two years in a health care environment
- Excellent customer service and communication skills
- Strong computer skills, including Electronic Medical Records experience
