Gila River Health Care is Hiring a Patient Liaison Near River, AZ
Overview
Provides bedside assistance to hospitalized patients in external hospital facilities by providing resources to counteract issues that negatively impact their healthcare. Assists patients and their families in scheduling follow-up appointments, referrals, patient advocacy for healthcare needs. Builds relationships with and markets to outside healthcare partners.
Responsibilities
- Assists high risk or elderly patients in local hospitals with preparation for discharge.
- Ensures patients and their family members are provided with the appropriate guidance and resources, including transportation services and support groups.
- Refers patients and their families to other GRHC department programs as necessary.
- Informs other involved staff on advocate activities and patient issues that may affect treatment or psychosocial status of patient.
- Documents and completes referrals and follow-ups as necessary.
- Provides home visits to patients as needed.
- Participates in related community activities such as health fairs and various departmental programs.
- Performs administrative duties as requested in the preparation for visitation.
- Communicates with patients and clinical staff to ensure patient compliance with prescribed treatment plan.
- Answers all incoming inquiries from patients, providers, staff, and others related to all aspects of practice operations.
- Records and enters accurate mileage log for fuel usage with receipts.
- Performs other job-related activities as requested.
Qualifications
- Bachelor’s Degree in Social Work or a closely related field of study required; Master’s Degree preferred.
- 1 year of experience in a clinical, social service, or community program setting required.
- Gila River Indian Community or Ak Chin Community member preferred.
- Current and continuing BLS training and certifications.
- State of Arizona driver’s license with no significant violations during the last 5 years.
- Demonstrates ability to relate to diverse cultures and specifically the Gila River Community or other Native American cultures and community health services.
