About PACE

Philanthropists Gary and Mary West founded West Health with the mission of enabling seniors to successfully age in place with access to high-quality, affordable health and support services that preserve and protect their dignity, quality of life, and independence. The Gary and Mary West PACE (Program of All-inclusive Care for the Elderly), affiliated with West Health (www.westhealth.org), is a new non-profit that delivers medical, social, and behavioral services to the frail elderly to the North County of San Diego. This PACE center will be entrepreneurial in nature and will focus on innovations and bringing new processes to the traditional PACE environment.

We're looking for passionate healthcare providers who want to be part of a team dedicated to enable seniors to live in their communities as independently as possible by providing compassionate, comprehensive health and supportive care.

Position Summary:

The Gary and Mary West PACE Physical Therapist Assistant is responsible for implementing and completing the restorative and rehabilitative Physical therapy plan of care. These services are provided to participants at the Center and at home, working in close coordination with PACE Physician, rehabilitation staff and other interdisciplinary team (IDT) members to maximize participant independence and safety and develop or restore function. The Physical Therapy Assistant collaborates with the Physical therapist, and Occupational Therapist to efficiently operate Gary and Mary West PACE rehabilitation department and, also, works effectively with colleagues and stakeholders to promote the Gary and Mary West PACE values, team culture and mission.

Essential Functions

· Implement Physical Therapy treatments in accordance with the physical therapist assessments, plan and recommendations.

· Deliver and document Physical Therapy interventions in accordance with physician orders and/or as agreed upon in the participants' care plans including but not limited to providing active and passive exercise, muscle re-education, gait and functional training of ADLs; treatments such as massage, heat, sound, water light or electricity.

· Provide training and instructions to participants and caregivers/families in the use and care of therapeutic equipment.

· Assist rehab staff with ordering, tracking and managing maintenance for equipment.

· Remain alert to pertinent input from other team members, participants, and caregivers and update the IDT promptly of any changes in participants' condition or functional status.

· Work with the other members of the IDT to support smooth care transitions between settings (e.g. hospitals, skilled nursing facilities, home, etc.)

· In coordination with the Marketing Team, support enrollment of prospective participants into the program.

· Participate in end-of-life care coordination and support.

· Complete all required documentation in a timely and accurate manner.

· Protect privacy and maintain confidentiality of all company procedures and information about employees, participants and families.

· Follow Gary and Mary West PACE policies and procedures and participate in any required Quality Improvement activities, staff training and meetings

· Other duties as assigned

Education and Certification

· Valid Physical Therapy Assistant license in the state of California, or proof of license eligibility; and

· Proof of Physical Therapy Assistant degree from an accredited institution.

· The qualifying Physical Therapy Assistant degree earned outside of the US must be evaluated to the US equivalent to a degree in Physical Therapy

· Current Basic Life Support (BLS) /First Aid Certification required

· Current driver's license and proof of auto insurance required.

Skills & Experience

· Minimum of two years Physical Therapy Assistant experience in a clinical setting with a frail or elderly population.

· Physical Therapy knowledge and skills necessary to treat frail, elderly participants with complex needs.

· Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.

· Excellent clinical, organizational and communication skills in settings with seniors, their families and interdisciplinary team members.

· Experience and competency working with people from diverse backgrounds.

· Commitment to unlocking the full potential of our most vulnerable seniors

Medical Clearance:

Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact. Job offers are contingent upon a successful pre-employment drug screen, background check, and physical assessment. COVID-19 vaccination is required
