We are searching for experienced, caring, & dependable applicants to join our family!
Responsibilities
Develop social assessment and care plan, which identifies medically related social and emotional problems and needs with realistic goals and specific actions to be taken.
Maintain grievance process and logs to positive resolution.
Participate in Utilization Review and other committees as directed and needed.
Assures that notes are informative and descriptive of services provided and of residents’ responses to services.
Prepares and completes social service documentation in timely manner.
Refer resident/family member to appropriate social service agencies when facility does not provide services or needs of resident.
Develop a social history on each resident.
Completing sections of the MDS.
Coordinating room changes.
Participating in behavior meetings.
Develops discharge plan at the point of admission to provide direction for the resident's stay.
Qualifications
BSW or MSW
Knowledgeable of social services practices, procedures, laws, regulations and guidelines governing long-term care
Previous long-term care experience is required. Past supervisory experience preferred.
