G&A Partners' client is currently seeking an Occupational Therapist to join its team in San Antonio,TX.

Provides occupational therapy evaluation, remediation, and training in the areas of sensory /motor skills, perceptual / cognitive skills, and independent living skills. Independent living skills range from basic self-care to meal planning and driving readiness. Assists the patient with transition to home and community living.

Responsibilities

Responsibilities:

• Effectively creates and delivers occupational therapy programming; documents and reports patient status to treatment team and case manager, and develops appropriate intervention strategies.
• Creates occupational therapy treatment plan based on assessment, assessment, documents findings, and communicates patient information to Case Manager in a timely fashion.
• Engages patient in goal-directed activities to achieve treatment plan objectives and program outcomes.
• Completes evaluation, creates and implements treatment plan, monitors and documents patient response to interventions.
• Implements treatment in the following areas: motor, sensation, vision and visual perception, provision of adaptive equipment, home modifications, return to driving and productive activity.
• Plans and coordinates appropriate activities to meet social / recreational needs through groups and community organizations..
• Provides practical training in higher order independent living skills such as menu planning, home safety and management, grocery shopping and budgeting.
• Develops therapeutic activities targeting driving readiness and medication-management programs.
• Participates in patient discharge planning including long-term wellness, health management and community re-integration.
• Provides resources for appropriate equipment, assistive technology, transportation, financial and care services prior to discharge.
• Provides education and training for the patient, family, and payer as requested by the Case Manager
• Demonstrates the necessary written and verbal skills to communicate formally and informally about the treatment plan effectively with other professionals, patient and family.
• Completes occupational therapy evaluation, progress notes, discharge summary and treatment notes in a clear, concise and timely manner.
• Maintains patient, family, physician and payor satisfaction with the individualized occupational therapy treatment plan.
• Effectively engages patients and families in pursuing treatment goals, activities reflect understanding of strengths and weaknesses.
• Participates in staff training and program development in areas related to occupational therapy.
• Provides formal / informal staff instruction in occupational therapy related areas.
• Develops and implements creative, goal directed programs in occupational related areas.

Qualifications

Qualifications:

• Bachelor’s degree in occupational therapy.
• Registered and licensed, where appropriate.
• Relevant experience.
• Abilities necessary for transport, transfer and management of patients.
• Demonstrates effective verbal and written communication skills.
• Agrees with the overall goals, objectives and methodologies of the program and Learning Services.
