G&A Partners is Hiring a Case Manager, Rapid Rehousing Near Kankakee, IL

Overview

G&A Partners' client is currently seeking a Case Manager, Rapid Rehousing to join its team in Kankakee, IL.

Responsibilities

  • Provides screening, assessment, case planning, and case management and aftercare services to resident families in the Transitional housing and Rapid Rehousing Program
  • Prepares and maintains accurate confidential case records, unit inspection reports, progress notes, fiscal documents and other supporting documentation.
  • Monitors participant progress and assist them with individualized goals and /objectives towards housing stability and self-sufficiency skill maintenance. (This includes home visits.) Documents services provided ensuring clarity and completeness of information.
  • Assists in special projects such as Holiday Outreach (Kathy’s Gifts program), Back to School Fair, and Mobile Food Pantries.
  • Assists with Program Quality Initiatives and some Administrative support functions, including reviewing files, filing, answering phone calls, covering for absence of receptionist.
  • Advocates, networks and engages with current and prospective landlords to create opportunities for permanent housing for clients, locates rental units in the community, and supports Rapid Rehousing activities for clients. 
  • Addresses unit safety needs and unit turnover needs including minor repair and maintenance, and cleaning needs. Meets contractors and other repair or extermination professionals at the units as needed. 
  • Assists with donations management, organization and client basic supply needs.
  • Assists with client inquiries, including referrals for services in the community.
  • Assists with Homeless Prevention cases including screenings, case management, and landlord engagement.

Qualifications

  • A Bachelor’s Degree is required for this position, A Bachelor’s degree is preferred with preference in areas of, Human Services, Social Work, Psychology, Sociology, Family and Consumer Sciences. All degrees must come from a regionally accredited educational institution which has U.S. Department of Education approval.
  • A minimum of 1 year of experience in Human Services areas is required.
  • Demonstrated ability to work effectively and constructively with persons from all racial, diverse cultures, family systems, and social backgrounds.
  • Proficiency in the use of Microsoft Office applications including Outlook, Word, and Teams.
  • Strong verbal and written communications skills.
  • Passage of complete background clearance, physical and Tuberculosis (TB) Test.
  • A valid driver’s license, reliable means of transportation and proof of liability insurance is required.
