JOB TITLE Virginia RN Case Manager

SUMMARY:

Provides professional case management and nursing services to enable identified individuals to make informed choices about opportunities and services, assuring timely access to needed assistance, providing opportunities and coordinating all services to meet each consumer’s goals and thereby assisting them in achieving their maximum level of independent functioning. Will also provide crisis intervention services.

EDUCATION AND EXPERIENCE:

Education: Graduate of an accredited school of nursing required. Bachelor’s in nursing preferred.

Licensure: Registered nurse licensed in Virginia.

Certification: N/A

Experience: A minimum of three years work experience in behavioral health preferred.

Knowledge/Skills: Verbal/written communication skills.

Ability to maintain composure in difficult situations.

Ability to safely operate motor vehicle.

CPR training

EQUIPMENT:

Computer, fax, copier, motor vehicle and any other equipment required to perform the functions of the position.

MAJOR DUTIES AND RESPONSIBILITIES:

1. Conduct interviews with individuals and/or family members in a therapeutic manner so as to obtain critical and thorough information, provide written documentation of information obtained and exercise good judgment in evaluating situations, making decisions and implementing effective strategies.

2. Provide assessment, service planning, crisis assistance, daily living assistance and linkage, referral and advocacy to/for referred individuals.

3. Provide written documentation to complete all necessary chart work in accordance with

policy on admissions, treatment, and discharges in a timely manner.

4. Maintain client confidentiality according to agency policies.

5. Remain current in knowledge of community resources and how to access those resources.

6. Keep supervisors apprised of all critical or emergency situations.

7. Respond to emergency/crisis or walk-in situations calmly, rationally and in accordance with agency policy and procedures.

8. Participate in inservice training and orientation and provide consultation and education services to the community as requested by supervisor/administration.

9. Be involved in the admission, hospital stay and discharge of individuals on caseload who are admitted to an in-patient psychiatric facility.

10. Be available to consumers 24 hours a day for crisis assistance. After hours, the agency’s after-hours system may be utilized; however, the on-call staff as appropriate may contact the Case Manager.

11. Must be able to safely operate a motor vehicle to ensure the delivery of community-based services.

12. Attend and participate in regularly scheduled staff meetings and inservices and individual program planning staffings, as needed.

13. Administer prescribed medications by injection. Perform venipuncture as ordered.

14. Counsel consumers on medications, therapeutic effects, and monitor/document positive and negative drug reactions.

15. Respond to physician ordered nursing duties.

16. Provide nursing coverage for medication clinics.

17. Provide preadmission screening services, as certified.

18. All other duties as assigned.
