Description
The Caregiver performs various personal care services as outlined on the care plan/service plan as necessary to meet the patient’s needs. The Caregiver will be assigned in a manner that promotes quality, continuity, and safety of a patient’s care.
KEY RESPONSIBILITIES AND ESSENTIAL FUNCTIONS:
· Follows the Patient’s Care Plan/Service Plan.
· Provides personal care, including:
· Bathing
· Oral hygiene
· Shampoos
· Bed linen changes
· Assisting the patient with dressing, undressing and personal hygiene
· Assisting the patient with toileting activities
· Keeping the patient’s living area clean and orderly, as appropriate
· May prepare a light meal during the visit.
· Assists in feeding the patient, if necessary.
· Provides medication reminders.
· May provide laundry services during the visit.
· May provide homemaking services, companion/sitter services, transportation and escort services and related medical activities as Meets safety needs of patients and uses equipment safely and properly (foot stools, side rails, etc.)
· Follows all privacy policies of the Organization, Infection Control protocols and maintains the confidentiality of Protected Health Information (PHI).
· Attends in-service training and education sessions, as assigned.
Performs specific work duties and responsibilities as assigned by supervisor.
Requirements
· Must meet the training requirements in accordance with State and Federal laws. A Certified Nursing Assistant (CNA) and/or Certified Home Health Aide may be hired as a Caregiver but will be governed by the same essential job functions as the Caregiver.
· Minimum of one (1) year of hands-on work experience is preferred. Can include experience in a home care, hospital or assisted living setting.
· Must be at least 18 years of age.
· Must be self-directed and have the ability to work with little direct supervision.
· Must have an understanding of the needs of the populations served.
· Must demonstrate tact, patience and good personal hygiene.
