Friends of Youth is Hiring an Intake Coordinator Near Kirkland, WA

GENERAL DESCRIPTION

The Intake Coordinator develops, implements and oversees the referral and intake process for all single young adults and young families in Transitional Living and Supported Permanent Housing programs at Friends of Youth. This employee acts as Friends of Youth’s liaison to King County’s Coordinated Entry for All (CEA) system, managing all aspects of the referral process for Friends of Youth’s 92 homeless housing units, including updating the HMIS database and managing data in HMIS, collecting housing referrals, conducting phone screenings and performing criminal background checks. The Intake Coordinator collaborates with community partners to verify eligibility and collect homeless status documentation, conducts outreach to referred applicants to provide information about our housing program, and connects applicants with prospective housing case managers for enrollment into services. The Intake Coordinator manages the Central Intake Line, triaging crisis calls and providing critical resources and support, and schedules and conducts in-person assessments for individuals seeking housing to facilitate connection to CEA. The Intake Coordinator is responsible for training new Homeless Youth Services staff to complete CEA assessments and vacancy reports, and supporting the team to utilize the referral and intake process effectively.

REPORTS TO: Housing Support Services Program Manager

JOB RESPONSIBILITIES

  1. Service Delivery
  2. Coordinate referral and intake process, submit openings to CEA system for referral, oversee status of unit openings and monitor for length of time in system, maintain internal waitlist for eligible applicants and provide details to denied applicants.
  3. Attend bi-weekly Case Conferencing Meetings for Single Young Adults and Families.
  4. Provide support and information to referred applicants to understand program model, requirements and expectations, collaborate with service providers to collect and verify required eligibility documentation, and connect applicants with housing case manager to schedule intake appointment.
  5. Proficiently utilize Washington State Judicial Information System and Washington State Patrol Access to Criminal History and National Sex Offender Public Website systems to conduct criminal background checks for all referred applicants.
  6. Manage multiple referrals for all open or upcoming vacancies, ensuring that all Intake Policy timeline goals are met for each applicant referred, including completing initial contact within 24 hours of referral, completing phone screening within 48 hours, scheduling intake appointment with case manager within 7 days to ensure move-in within 14 days of referral.
  7. Manage the Central Intake Line, triaging crisis calls and providing resources and support as needed. Return calls of inquiry and provide relevant, accurate information about our programming and services. Provide screening forms and access to our internal waitlist to potential applicants.
  8. Complete updated Current Empty Units Report and distribute communication to housing program service providers weekly. Complete updated Matrix report and distribute to program supervisors and managers weekly. Submit weekly update report to program manager.
  9. Attend weekly supervision meetings, monthly team meetings and division meetings as required.
  1. Program Operations
  1. Collaborate with staff across various programs to create cross-agency engagement and partnership (ex. Housing Navigation, Drop-In, Landing, Transitional Living, Supported Permanent Housing)
  2. Collaborate with Program Managers and Program Supervisors to effectively communicate status of all current vacancies, pending referrals and upcoming openings.
  3. Act as Friends of Youth’s primary liaison and first point of contact with CEA to ensure a positive team approach. In conjunction with CEA, provide reports for current or upcoming vacancies; request, collect and process referrals for available units; and provide follow-up communication as needed.
  4. Maintain T3 Trainer Certification with CEA, and provide training to new assessors to conduct assessments, access HMIS and input assessment information accurately.
  5. Conduct and support training of Housing Case Managers and Housing Navigators in intake process and completion of weekly vacancy reports.
  6. Develop, implement, and expand policies to increase system responsiveness for referred applicants.

  2. Develop and maintain a referral network of homeless housing providers to ensure updated information on accessing available resources.
  3. Work in conjunction with Program Manager to advocate on behalf of homeless youth, families and system changes within King County and specifically with the Coordinated Entry for All system.
  4. Participate in professional group seminars and conferences to increase professional knowledge of current best practices and issues impacting youth experiencing homelessness.

JOB REQUIREMENTS

Education and Experience

  1. Bachelor’s degree in social work, psychology, counseling or related field or 2-5 years of relevant work experience required.
  2. Experience working in social services required.
  3. Experience working with diverse populations required.
  4. Experience working with the homeless population preferred.
  5. Education or experience with case management preferred.
  6. Bilingual/bicultural preferred.

Additional Requirements

  1. Must satisfactorily pass criminal history check.
  2. Must satisfactorily pass tuberculosis (TB) test.
  3. Must successfully complete the following health certification requirements within 30 days of employment: CPR, First Aid and Blood borne Pathogens training.
  4. Must have reliable transportation and a valid Washington State driver's license (or be able to obtain one by date of employment).
  5. Employees and volunteers who operate their own or Friends of Youth vehicles on Friends of Youth business must have a safe driving record as defined by Friends of Youth.
  6. Employees and volunteers who operate their own vehicles on Friends of Youth business must carry auto liability insurance that complies with the requirements of Friends of Youth.
