JOB TITLE: Direct Support Worker

PROGRAM: Community Wellness Psychiatric Rehabilitation Program, LLC

EXEMPT/NON-EXEMPT: Exempt

SUPERVISED BY: Program Rehabilitation Program Director / Rehabilitation Specialist

COMPENSATION: $12.50 - $18.00 per hour payable biweekly commensurate on experience.

WORK SCHEDULE:  Monday – Friday from 8:00 AM to 5:00 PM. 

ANTICIPATED TRAVEL: 15% of the time. There is no additional compensation for travel.

SUMMARY OF POSITION RESPONSIBILITIES: Under the supervision of the Psychiatric Rehabilitation Specialist, growing psychiatric rehabilitation program is seeking qualified competent direct service psychiatric rehabilitation workers to service clients in the community group living arrangements on an independently contract basis for immediate hire. In collaboration with the housing administrator, the worker will be responsible for managing the day-to-day operations of the housing unit in which the assigned client's reside. The worker will help to manage clients' treatment schedules for outpatient mental health clinic and primary care services, operate nurse practitioner originating site equipment for telehealth based psychiatric, and help clients participate quality assurance sessions. Structured direct support service encounters are to take place during the first 6 days of each month and documentation submitted by the 7th day of that month. Non-structured support encounters may take place for the remainder of the month while on duty. Worker may need to be available to assist client's in the event of a crisis. Worker's shall ensure client's receive all prescribed medications while monitoring the client's ability to self-administer / secure those medications frequently throughout the month providing assistance where necessary. Several clients cope with co-occuring mental health and substance abuse disorders or have a history or substance abuse; for these clients, worker will be asked to collect urine samples from client on a regular basis and process those samples with the program's preferred processor.

Community Wellness Center is a community mental health service provider that tailors specialized therapeutic treatment services to emotionally and behaviorally challenged youth, adults, and families. Our Psychiatric Rehabilitation Program provides community-based services that improve the quality of life for individuals with mental health challenges and social impairments. Our strengths-based rehabilitation services are designed to produce healing and restoration in communities. A direct support worker is expected to facilitate psychiatric rehabilitation, behavioral support services, and crisis intervention for assigned clients, according to Individualized Rehabilitation Plan, by:

  • teaching and instruction
  • encouragement of positive change
  • skill demonstrations and practice
  • promote individual wellness, self-management, and recovery
  • assist in the restoration and/or improvement of self-care social skill
  • assist with access to entitlements
  • facilitate effective strategies for strengthened independent Living and the achievement of self-sufficiency goals
  • Expose individuals to academic, vocational, and employment training and resources
  • Facilitate culturally relevant activities directed toward improved social relationships
  • Encourage Health Promotion, Proper Nutrition, and Exercise. 

Responsibilities and Competencies

  • Provide direct support worker services to assigned clients as scheduled by the Company (supervisor). Caseload shall not exceed 45 clients. Must document each encounter in the direct support worker online portal on the same day of the encounter by the close of business. Worker may set schedule to meet each client independently.
  • be on duty in the office Monday to Friday 8a-5p and available on call to clients after hours one hour break is from 12-1 each day
  • manage the day to day housing operations and property management needs on The property in which your group living clients reside
  • attend staff meetings once per week or as scheduled
  • Responsible for carrying out services and supports that meet the clients medically necessary mental health needs reflected in the client's individual rehabilitation plan (IRP) promoting independence, freedom of choice and overall wellness
  • collect urine samples from each client as directed weekly
  • Must collaborate with the client's therapist, nurse case manager, mental health case manager, psychiatrist and other persons involved in the client's care to ensure the client complies with treatment schedules on a monthly basis (in some cases weekly)
  • Assist the client with monitoring and obtaining access to all entitlement benefits for which the client may be eligible and monitor benefit status on a monthly basis helping the client to apply when necessary
  • Be available to clients in the event of a mental health crisis while on duty and after hours
  • If the client resides in a community based group living arrangement, assist the client with needs as it pertains to said arrangement in addition to other psych rehab needs
  • Carry out any other work related functions requested by the supervisor
  • Ensure clients have an adequate supply of medications and monitor medications on a regular basis for each client
  • Document incidents, hospital admissions in contact note
  • conduct room inspections weekly

  • You will collaborate regarding your schedule with the Rehabilitation Specialist and ensure you complete the following additional duties:(a) manage the tele-medicine system when clients in your caseload are scheduled to receive treatment via tele-medicine with OMHC providers; (b) collect urine/saliva samples from clients for medication and substance abuse monitoring as needed; (c) be available during community housing unit inspections and ensure clients manage housing units according to posted policy; (d) receive food and supply deliveries; (e) assist registered nurse case manager; (f) collaborate with client's therapist and all persons involved in the client's treatment at least monthly
  • Are responsible for implementing the rehabilitation activities outlined in the adult or minor's IRP.
  • If assigned on an hourly basis in an assisted living facility, also serve in the capacity of "Direct Care Staff" while on duty.
  • If assigned on an hourly basis in a residential treatment facility or supportive housing facility, also serve in the capacity of "House Manager" while on duty.
  • Ensure the manager has valid unexpired contract vetting requirements on file with the Company at all times.
  • Participate in external and internal audits/surveys (CARF/CSA/OHCQ) as directed by supervisor.
  • Participates in quality assurance and performance improvement plans by completing periodic audits or other activities to ensure regulatory compliance and/or improve service delivery.
  • Collaborate with and serve as program POC for external treatment providers and outside agencies as needed.
  • Responsible for following regulations of COMAR, CARF, any other regulatory body, and company policies/ procedures related to service delivery and documentation.
  • Support the maintenance of a safe environment by participating in drills as requested.
  • Advise the supervisor on the development and implementation of protocols to better enhance the efficiency of day to day operations effecting change when approvals are made.
  • Troubleshoot and abate any issues that could adversely affect the day-to-day operations.
  • Report to the Supervisor daily and as requested. Report deviation of operational standards to CEO daily.
  • Collaborate with department leaders, external landlords, vendors, utilities providers, housing related external entities and supervisees regularly as needed.
  • Have knowledge of Policies and Procedures of Holbrock Estates Supportive Housing / LightHouse Behavioral Healthcare.
  • Maintain a working knowledge of COMAR as it pertains to assisted living and other residential regulations within the scope of services rendered by the company.
  • Report to the ALM, CEO, COO, manager via email when starting and ending service period if not using Company’s time keeping system.
  • Check emails and company group texts at least every 30 minutes while on duty; respond accordingly.
  • Assist the ALM, CEO, COO and/or contract manager with other services as requested / directed.

Skills

  • Proven working experience in case management, including as a nurse, medical, mental health care manager or a related job

  • Posses a basic knowledge of all office equipment, including word processors, typewriters, Dictaphone, FAX, and copy machines, and a willingness to attend appropriate classes to increase knowledge as needed.

  • Must demonstrate you are free from TB, measles, mumps, rubella and varicella
  • Must pass a CJIS criminal background check and have no criminal convictions or history that indicate behavior which pose a potential threat to clients and staff
  • Ability to follow CWC Clinic practice guidelines – including adhering to the CWC immunization policy and the CWC Mission and Vision and CWC Corporate Compliance Policy.
  • Must be 18 years or older
  • Once hired, must complete 40 hours specialized PRP training before servicing clients independently and an additional 20 hours direct supervision if rendering services to minors

LEVEL OF EDUCATION/TRAINING/QUALIFICATIONS:

  • It is preferred (but not required) that the candidate possess at least an associate degree in counseling, psychology, social work, or health related field (Interns and students pursuing such degrees may provide services under this scope through contract with the higher learning institution only)
  • 1-year experience in case management, mental health counseling or substance use disorder related counseling
  • High School Diploma/GED; AA Degree or greater preferred.
  • If servicing minors: 1. A bachelor's degree in a health-related field; 2. An associate's degree in a health-related field; 3. 30 hours, or their equivalent, of college credit toward a bachelor's degree in a health-related field; or 4. 1 year of work experience in a supervised mental health setting; Are supervised by a licensed mental health professional or a rehabilitation specialis; ii. Have 60 hours of on-the-job direct PRP supervision before providing services to minors served by the program without direct supervision
  • Minimum two (2 ) plus years of experience providing direct support to adults with severe persistent mental illness and substance abuse disorders. These will be verified through your professional references provided.
  • Current first aid and CPR certification

REQUIRED CERTIFICATION/LICENSE:

  • Must have a driver's license and your own reliable transportation
  • Must be a certified medication technician (CMT) listed on the MBON website and demonstrate competencies to the RN CM prior to handling client's medication
  • It is preferred that the candidate possess a certified nurses aide (CNA) license
  • It is preferred that the candidate is a licensed social worker, licensed/certified/approved counselor, registered peer recovery specialist

PHYSICAL DEMANDS: Regularly walk, stand or stoop; occasionally lift, carry, push, pull or otherwise move objects weighing up to 25 pounds; and regularly drives a motor vehicle.
