Overview

Mount Alverna Village, a part of Franciscan Ministries, is hiring a Full Time Wound Nurse. This person would be responsible for overseeing the treatment of all residents with pressure ulcers as well as be available as a resource for all other skin integrity issues as needed.

Mount Alverna Village is a vibrant senior living community in Parma, Ohio. Mount Alverna Village is a continuing care retirement community (CCRC), offering a variety of senior lifestyle options including independent living, assisted living, and skilled nursing/long-term care. Additionally, the onsite rehabilitation clinic offers inpatient and outpatient physical, occupational, and speech therapies, and state-of-the-art programs designed to enhance the quality of life for residents and patients in the local community.

Duties and Responsibilities:

• Oversee and assess all residents with pressure ulcers.
• Assess all residents with skin integrity issues as consulted.
• Collaborate with physicians to develop a plan of treatment during weekly wound rounds.
• Oversee the administration of treatment to the resident.
• Administer treatments per physician.
• Assess the effectiveness of the treatment, document weekly in the resident chart any progress and changes including weekly documentation.
• Notify physician with non-healing, non-improving wounds and as needed.
• Communicate with unit managers and Director of Nursing any residents that are at high risk for skin break down, non-healing/non-improving wounds as needed.
• Establish would care protocols including identification of high-risk residents to promote optimal skin integrity outcomes.
• Identify associate educational needs related to wound care and coordinate in-services to meet the needs of the nursing associates.
• Prepare weekly skin report and present to DON. and Administrator.
• Report on skin outcomes monthly at monthly QAPI meeting.
• Act as a clinical resource for associates.
• Assist with in-house audits as needed per request of the DON.
• This role will participate in the nurse manager on-call rotation.

Required skills and qualifications:

• Graduate from an accredited school of professional nursing.
• Current LPN or RN license to practice in the State of Ohio
• CPR Certified
• Minimum 1 year supervisory experience preferred.
• LTC and skilled nursing experience preferred.
• Recent clinical experience, education, and specialty skills specific to geriatrics and would care
• Flexible schedule, including availability to work nights, weekends and holidays as needed when on call
